In mid-February 2017, UNM will mail a Form 1095-C to benefits-eligible employees that details information about UNM medical coverage held or offered during 2016.

The form is used to verify employee coverage each month that the employee was enrolled in UNM’s medical plan in 2016. Benefits-eligible employees should confirm their mailing address in LoboWeb.

While the Form 1095-C can assist you in preparing your tax returns, it is not necessary to wait to receive it to file your individual income tax return. IRS guidance allows individuals who do not receive the form before filing tax returns to verify insurance coverage by using other forms of documentation.
